I have broken the data down into Tables that represent the various sections of the field form.  The CMRA table represents the admin section, the CMRB Table represents the location section, the CMRC table represents the selected summary data except for the Habitat and Community types, the CMRCHab table represents the Habitat and Community types, the CMRE table represents the Photo data and the CMRScore represents the Scores.

There was not Water Quality or Weed data collected for the CMR data and so there are not tables for them.

The fields are in the table corresponding to the field from from top to bottom, if there are more than one item on the line, then from left to right.  For example, it is in the order of A1, A2, A4, A9, A12a, A12c, A12d, A12e, A12f, A12g.
